gcc/ChangeLog:
        * opts-diagnostic.cc: Apply renamings throughout for clarity: from
        "name" and "format_name" to "scheme_name", from "m_format" to
        "m_scheme_name", and from "handler" to "scheme_handler".
        (output_factory::scheme_handler::get_handler): Pass name by const
        reference.
