I have no particular knowledge of these target hooks, but doc/tm.texi is a generated file that is created by sucking the doc strings in target.def into the skeleton in doc/tm.texi.in. You should not be editing it directly to update for changed target hooks, just regenerating it. If that's what you already did, your ChangeLog should just indicate that you did so instead of mentioning specific things you changed, like:

        * doc/tm.texi: Regenerated.
