I'm looking into this: apparently the problem is just (or mostly) that at the
beginning of instantiate_decl we are not copying DECL_IN_SYSTEM_HEADER (d) into
in_system_header: doing it fixes the test here (and avoids the warnings in
30463 and 30464) without regressions.
