Could that be related to modifier keys? These nested X window systems often have the problems that they notice that a modifier key has been pressen (shift, ctrl, num-lock etc.) but not that it's been released. When the problem shows up, focus the virtualbox window and hit all modifier keys once and see if that helps. Is FvwmIconMan running separately or inside FvwmButtons? > All the other bits of fvwm seem to be still working: title-bar buttons (so I > can iconify things, just not get them back conveniently), root-window menus, > FvwmPager.
