I believe the first and second patch are good to go.
As for the third patch, I think the png image can be improved a bit
more, by tweaking the fonts a little.
And for the fourth patch, I'd like to include
)set output length 88
in src/doc/htex2input.awk, because for those verbatim
"algebra stream" output, the default setting is a bit short.

There has been a few minor improvements in the past days,
and I've built the book many times without failure.
Note that in patch 3, I've changed the Makefile target
from "book.pdf" to "book".
