A bare bones install of DOS is extremely easy, but it's adding all the
extra cruft (and trying not to be too specific to certain setups) that
makes it harder. Also, a large dilemma is third-party programs (even
with FDNPKG), which all have their own rules and requirements and
dependencies. So obviously "installing" something else (Fractint?)
depends entirely on where it must go and whether you expect other
tools to know about it.

Not to toot my own horn too much, but my own MetaDOS .img was meant to
be ultra lean (maybe too lean) so that anybody could (in theory) add
everything else in later. Obviously it's not super useful by itself,
but I hardly see how it could get any simpler.

The few folks at FreeDOS don't maintain VirtualBox and (AFAIK) aren't
in close cooperation with them. There are no explicit guest extensions
or plugins to make it work better here.

I don't think that's a bug, but presumably just the way the VM works.
Granted, I don't use VBox enough to know everything, and usually I
avoid the mouse entirely, but a quick check with (textmode) FD Edit
and Ctmouse shows everything to work as expected. Check your settings,
or at least be more specific about what's happening and what you
expect.

I agree that FreeDOS has done a marvelous job (and Eric especially).
But it isn't very active in development anymore. It's mostly stable,
"good enough" for most people. So I wouldn't get my hopes up too high
for any major changes (nor bugfixes).

Presumably from here:
