You do know that the maximum legal size for a FAT16 partition is 2GB?

NT Setup would create 4GB volumes then immediately convert them to
NTFS, but nothing much else will use 4GB FAT16 volumes, and that is
the absolute size limit. For actual /use/ it's 2GB.

For what it's worth, in the old days of DOS, I used to use Norton Disk
Doctor for recovering badly-damaged volumes. Scandisk is about the
next best thing, but NDD was considerably better. You can probably
find it as abandonware somewhere now. If you can't, ask me; if I can
find an old copy, I'll send it to you.
