I found this (lots of files in a directory) to be generally a Bad Idea. Search performance tends to be poor even with the improvements in 5.x
While I'm not sure that it's possible to in this instance, I have previously in a practical implementation, used OpenLDAP to specify separate 'Maildir' directories for mail storage based on the first letter of the username. Although I'm sure there are better and more distributed hash mechanisms, this was the quickest (and most commercially expedient) to hack into the LDAP database,
