To answer my own question, I used the php4-extensions port and selected
what I needed.
#cd lang/php4-extensions
#make config
selected the extensions I wanted
#portinstall php4-extensions

'--with-apxs=/usr/local/sbin/apxs' '--disable-ipv6'
'--prefix=/usr/local' 'i386-portbld-freebsd5.3'
So, then I tried it manually:
Shutdown apache
#cd /usr/ports/www/mod_php4
#pkg_delete -f mod_php4
#make -DEAPI--enable-calendar
#make install
Restarted Apache
Still the same options listed in phpinfo.php.
