Uhh --- look at the OP's supfile. See the line where it says
"*default base=/usr" ? He's overridden cvsup(1)'s default base, which
is the standard practice for all of the cvsup'able collections of
stuff from the FreeBSD project.
The OP. needs to move the refuse file to:
/usr/sup/refuse
