According to exim.org CVE-2018-6789 was fixed in 4.90.1. The port version is 4.90.1 so yes it should be fixed. see https://www.freshports.org/mail/exim/ for a handy overview. Looks like the quarterly pkg repo is also up to date from a quick glance.
