Hmm, I've never done a manual boot.catalog, always just ran mkisofs as so: mkisofs -b boot/cdboot -no-emul-boot -r -J -o foo.iso /some/dir and let mkisofs create its own boot.catalog. If you do that instead of creating one yourself does it work any better?
