This is report based on old data.
The rules were not loading because hostnames had crept into my pf macros. The
hostnames failed to resolve at load time (because DNS was not ready). Once I
fixed that problem, the rules loaded at boot time.
The error messages provided were from a 6 day old screenshot which I
incorrectly assumed was still a problem. It was outdated.
