Some work was done in the LCL-Cocoa and LCL-Win32 to support screen readers. Since LCL-Win32 uses native controls, it should be acessible to screen readers out of the box, but it would be great if you could test how the LCL apps behave in practice. I tested a long time ago.
About UI designing, I think it is best if you bypass the Form designer completely and write the UI in code instead.
