You might want to try Digital GEM Professional. It works as a Photoshop plug-in and you can make several adjustments, including controlling the grain in the shadows and highlights. It also contains an excellent sharpening/softening algorithm that is very useful.
You have the choice of two algorithms, Noise Suppression or Grain Suppression. I prefer Grain Suppression in most cases. However, Noise Suppression works better when grain is extreme. There is a Noise viewer (buttons for Before, After and Noise, it is the Noise button). It lets you see whether you are removing random noise (i.e. grain) or image detail. You can adjust to the point where you are losing image detail, or remove image detail that isn't wanted in the image (portraits for example).
