Not necessarily since in iso format it isn't in the same "state" (not
sure if this is the right word or not) iso format as I understand is not
unlike tar where when one file ends another immediately begins. It also
may contain a fair amount of other data to tell the computer how it's
laid out (Hash tables etc etc) that may not be needed on the CD itself. 
So from my understanding the fact that the md5's aren't the same is no
suprise.
