I would suggest copying the entire contents of the CD into a seperate directory on your C Drive (ie c:\mdkcd or similar), and then boot off a hd.img floppy (look in /images on the CD). When it asks you where the files are, select the drive (/dev/hda1?) and the directory (/mdkcd). I think it looks for files from the directory tree as on the CD (ie expects to find the same files in the path you ive it as it would when accessing the CD directly).
