I occasionally receive  such messages and EXMH just works.  The email is
has both  text/plain and text/html, and  both the text parts  are base64
encoded. When I reply in EXMH and then press <Control-q> to quote it, it
inserts the text/plain prefixed with > as the quote character.

Here's an example:

$ mhlist +inbox 3465         
 msg part  type/subtype              size description
3465       multipart/related          92K
     1     multipart/alternative      22K
     1.1   text/html                  10K
     1.2   text/plain                4931
     2     image/jpeg                 51K image001.jpg

Parts 1.1 and 1.2 are both base64 encoded:

--_000_CY4PR0401MB3602C06F6C597B086BA570E4C51B9CY4PR0401MB3602_
Content-Type: text/plain; charset="utf-8"
Content-Transfer-Encoding: base64
