After reconfiguring from scratch a few times narrowed it down to this error in the exim4 mainlog:

Not much to go on there apart from "it crashed". What was it doing at the time? Anything logged immediately before? If not, if you place a custom log line in the connect ACL, does it shoe up consistently before crashes? Can you run with debug?
