Exim does run a cache for lookups, but it also re-execs at certain points in processing (to regain privs, needed eg. for delivery to file) and that loses the cache. So it's complicated. In general you can't rely on a given lookup being done only once, or on being done every time. In practice: within ACL processing, only the first one.
