While we still should consider external influence as a root cause for
this database corruption, I got reports of a major Exim user, that
they're used to remove the hintfiles regularly, as the Berkeley DB isn't
that robust as it should be.

Of course, we should be sure, that it is not Exim that creates the
corrupt files by inserting invalid data (I didn't look into the source
right now. Is there some chance to have a duplicate of the
printing/unprinting thing?)
