The problem seems to be that in your workbook Company_Reval.xlsm, your consolidated sheet is missing column headings in column N and O All columns in your pivot table have to have column headings. looking a bit closer, it looks like your new file is missing the City column from column J, which then shifted the Zip/phone/shipVia/Company columns. it's allso missing the "HS" column. adding the necessary column headings should fix your problem.
