The text can be formatted by having the space *within* the format....

=CHOOSE(A1,C2,C3,C4,C5)&TEXT(C10/I1," 0,0.00")

or using Don's suggestion

=INDEX(C2:C5,MATCH(D12,D2:D5))&TEXT(C13/INDEX(E2:E5,MATCH(D12,D2:D5)),"
0,0.00")
