Hi Ramesh,

 

Please find the file attached with the vlookup formula.

 

The basic formula is:

=VLOOKUP($A3,Sheet1!$B$4:$F$18,4,0)

 

The ISNA formula is to replace any blank cells which come up with NA# with 
'blank' cells.
