Is this about a POP account or about an IMAP account? 
Is there any potential server-side filtering involved?

I don't think that SpamAssassin relies on simple allowlists based on
alleged sender addresses only...

Have you checked if those custom headers exist in those messages ending
up in a Junk folder?
