Hi,
I think while removing/disabling the old account
gconf sync. didn't happen properly.
To remove the old accounts folders from Contacts and
Calendar components, you need to remove them from
apps/evolution/[addressbook/calendar/tasks] using gconf editor.

For GAL folder not appearing issue, please run
evolution-exchange-storage from console and see if there are 
any log messages printed.

2.4.x has many issues, which are fixed in 2.6.x.
If possible, it is better to use 2.6.x version.
