What do other manufacturers do ? Tesla, for example ? In Nissan's case,
can the DC-DC be inside the battery box and, therefore, run while the
contactors are open ? If this were done, would it eliminate the need for
a 12v battery ? Could there be a switch for setting the car to
"extremely low parasitic power draw", for use while you are going to not
use the car for a long time ?

The reason the DC2DC converter does NOT run all the time is that (for safety)
Nissan has chosen to disconnect the HV battery from the rest of the car (with
contactors inside the main battery) whenever the car is turned off. [Meaning
that the ONLY place in the car where HV is present is inside the bolted shut
steel battery when the car is not in operation.
I feel this is a good engineering choice (but it wouldn't be a bad thing to have a
"if the 12v battery gets low, automatically turn on the car and charge it back up
for a while" mode that kicks in every few months if the car is idle....)
