The new ExtensionSettings policy will reinstall an extension if it is uninstalled.
As far as updating an extension goes, I'm still working on figuring out the best way to do that since we don't have version information. There currently is no way to change the update server though. You would have to repackage/resign the addon and add an update URL pointing to your own server.
