Great point Mike – yes the issue only manifested on new installations for us! 
That is why then – existing users had it migrated automatically by Firefox.

My guess is that because your sqlite file was already prefilled, it got 
migrated to the new format.
If you are explicitly adding the entries, you need to add both.
