This draft addresses the final open issues. I've updated the github repository to verify and close the open issues.
The only remaining open issue is verify against the errata. I did that months ago, but haven't double-checked it recently. I very much wish for this document to be done, and quickly. Thanks to everyone for helpful and detailed reviews.
