This version has all references to PAC removed, other than:

* a note that PAC was defined in RFC 7170 but is no longer used.

* updating the IANA registries.

  It also contains a few more clarifications resulting from the last interim.

  I think there are a few pull requests to merge, and then the document should 
be ready for WG last call.
