For many-off square holes the same size make a square broach:

For one-off squares holes in a variety of sizes i would:

1) Set up the part in the 4-jaw chuck and bore to AF size.
2) Use the chuck jaws to index into 4 positions
3) Use a CNC lathe to "slot" in the corners using a bit of square tool
steel smaller than the bore size

Though, actually, what I would do is cut a slot in the top and rely on
two flats only to do the work.

In fact, what I would really do is use one of my floating tap holders.
That forgives a bit of slop on the Z-axis better.
