Just talked to the operator, he said he would rather have all three options, so I told him they were either going to have to remember to add an M28 " Thru-spindle Off" to shut it off in the programs, or modify the post processor they use to include this with every M09 issued.
Maybe I will put in a feature request to allow minimal remapping of M09, or add a thru spindle coolant option in general.
