I would agree with Dan, looks like a notation for a heatsink.
The pinout looks like a surface mount MOSFET so I would guess the meaning as "requires heatsinking / thermal spreading as part of PCB structure" If part of a DC/DC converter or power supply this is a good way to connect a high dV/dt node to one plate of a capacitor!
