I didn't think hard enough on this to have a steady opinion, but I guess the right thing is to extend appt.el facilities, not org.el directly. The job of `org-agenda-to-appt' is to make the appointments list (stored in `appt-time-msg-list') aware both of the diary and of agenda files.
