I have spotted similar problems starting with 091 (the first version of Elephant I used) but could never pinpoint them. I suggest that like me you upgrade to the current Elephant 1.0 as a next step to make sure that this hasn't been fixed in the meantime. After that we could add basic index sanity checks in the slot index code to find the source of the error.
