I have the same issue here, Ubuntu 14.04.1 64bit (3.13.0-33-generic) with Unity, using Dual screen and NVS 300, Nvidia proprietory drivers v 340.24.
Sometimes in the past I was able to get the input password box to appear when I was moving the mouse around from one screen to another. This was causing the login box to change screens and sometimes the input would come back. Lately this doesn't work for me any more, and the only work around is the unity --replace command from a terminal. I also noticed that when I come back to my session my gnome-terminal is completely lost in the background and if it was minimized I cannot bring it back.
