Another scenario that appears to cover the same bug (See also: https://answers.launchpad.net/ubuntu/+question/237638) :
When I log in and then log out, on the login screen there is a small triangle before my user name indicating I am still logged in (although I no longer am). However this triangle stays there until I restart my computer. Even though the logged-in symbol in the session menu disappears in certain cases (see comment #4 above), the triangle before the user name on the login screen always stays there. It looks like whether the user is still logged in is determined in another way in the session menu (the logged-in symbol) and on the login screen (the small triangle).
