Titles for DCInput Pages

When the input-form contains more than one page then it might be useful to be
able to add an explanatory title to every page. For example, in my case I have
a input-form with three pages where the first page collects generic Dublin Core
metadata, the second page collects specific Dublin Core metadata (about the
previous publication of the item) and the third page collects custom (ie.
non-Dublin Core) metadata. So, by using page titles I can communicate better to
my users what I expect from them on every one of those three pages.
I created a nested class DCInput$Page for this purpose, and performed minor
refactorings here and there. I have integrated this feature with XMLUI but not
with JSPUI. Please see the attached patch file.
