That's a bug, but it's not very easy to see what's wrong.. v2.2 gives somewhat better error message about this. And the upcoming v2.2.6 will have even better error message here. Of course, having better error messages doesn't really fix the problem itself. I'm not sure but I think there are some fixes related to this in v2.2, so simply upgrading might help (although it won't fix any old attachment files if they were written wrong).
