This was actually a pretty rare problem that happened when two processes tried to create a previously non-existing Dovecot index at the exact same time, but they had a different value in "current time" variable (by one second). This'll fix it: http://hg.dovecot.org/dovecot-1.2/rev/1fb19e1c84f5 So next problem: the login process crashing..
