Just wanted to follow up and say that specifying the quotas in the
userdb has resolved the issue; maildirsize files are created if
they're missing and quotas appear to be handled as designed.

So thanks very much for the solution Timo.   This is my last week at
this job and that was my last major ticket.  Much appreciated!
