"Logged out" means that client sent a LOGOUT command.

I suppose this is the problem then. Outlook stupidly keeps reconnecting to server all the time, so if one reconnection fails I guess this problem happens.
So this "johnfowler" really should exist in your userdb? You could set auth_debug=yes and then see what is logged before that error. Also could you post your dovecot -n output?
