Mystery solved.

It does work as advertised but my testing was assuming that all admin
pages would be affected rather than just change_form.html etc.

So as it stands there is no way to override the admin index page on a
per-app basis using this mechanism. Not a problem for me at the moment
as the global override is enough but it was rather confusing during
testing. I'll add a comment to the book when I get a chance. I
couldn't see this functionality mentioned in the main docs anywhere.
