You may not need Django for this. You could just use Flask e.g. https://pythonspot.com/flask-web-forms/ and https://stackoverflow.com/questions/27379486/retrieving-html-form-data-and-storing-in-csv-with-flask-python
but, as with all web frameworks, you need to look at the fundamentals of how Flask works first (otherwise you copy-and-paste without understanding why).
