I believe it was a little custom script that generated a schema model in OmniGraffle from the ruby code. Not 100% sure, but I recall seeing one of the local ruby guys using it.

If you could point to the specific screencasts, that would make
identifying the tool easier.

At a guess, though, CocoaMySQL is a likely candidate. Even if it's not
what they're using, it's useful for the things you list:
