My favorite work on the topic is actually a presentation (and at one point an
invited talk at LISA apparently) done by Steven Levine (then of SGI,
currently of RH, IIRC) called "Why the Documentation Sucks and What You Can Do
About It." The presentation is available online at:

I was lucky enough to see him present this at an old MAD-SAGE meeting in
Madison, WI about the time he did this one at LISA.

It was much better in person, as Steven is quite entertaining and a great
presenter (it includes several songs and a bit of dancing), but the content is
golden on its very own.
