The UW-Madison Digitial Media Center (which just closed, but that is a different issue) had lots of equipment for checkout, including (for example) video cameras, tripods, etc.

They bar-coded *everything* (even USB cables, all the parts of the tripod, the carrying cases, etc) and scanned them out and in, and made the recipient sign (on a touch screen) for it. I think they also made some bundles or chacklists to assemble bundles, so that when I went to pick up equipment, I said I needed a camera, mic and tripod, they also had all the accessories ready.
