We take this approach, with an additional environment called preview which is customer-visible but is clearly marked as not for production use. Having more eyes is better. It's also important to have a clearly defined emergency policy, so that when an incident comes up -- say, a way to read any file via your web interface -- you already know who has to approve the immediate change to all environments both for business reasons and technically.
