Inkyu, At least in 3.9.4 that could be a sequence number error ( https://github.com/EttusResearch/uhd/blob/release_003_009_004/host/lib/usrp/common/async_packet_handler.hpp#L62). Odd that you're seeing that with USB. Not really sure how to help you with that part.
